Quality Clauses – DESCRIPTION OF CONTRACTOR QUALITY REQUIREMENTS
- CERTIFICATE OF CONFORMANCE: The supplier shall provide an original copy of a test reports or certificate of conformance including
those from sub-tier suppliers, with each shipment or after the completion of a service. The certificate of conformance shall be signed by
quality personnel or a supplier authorized representative. The report shall state compliance to the purchase order, specifications including
classifications when noted. As a minimum, the certificate must contain the following information as applicable: a report/certification number,
date of issue, name and address of the supplier, the Electrolurgy, Inc. purchase order, a description of the service provided, the product
name, batch number, quantity shipped, part number, the actual tests values, the manufacturing/test dates, expiration dates, the date of issue,
as applicable. All C of C’s shall be certified to the latest applicable specification revision, unless noted otherwise. The supplier must have on
file all material, chemical and physical test reports, C of C’s and all applicable documentation for processes or services subcontracted and
shall make this documentation available for review by Electrolurgy Inc. - QUALITY/INSPECTION SYSTEM: The supplier shall establish and maintain a documented quality system that complies with the
requirements of the government or the industry’s quality standards. When applicable, the seller shall maintain a calibration system meeting
any of the following specifications: ISO/IEC 17025, ISO 10012, ANSI/NCSL Z540.3 (Obsolete spec Mil-Std-45662). Unless granted
otherwise, the supplier shall maintain a minimum inspection system complying with the requirements of Mil-I-45208. The quality system is
subject to verification and approval at all times by Electrolurgy, Inc. Co. Quality assurance department. - NO CHANGES AND SUBSTITUTION: The seller shall not make changes to the products, specifications or the tests specified in the purchase
order, unless approved otherwise. Seller shall notify Electrolurgy, Inc. in writing on proposed changes to controlled products, test methods, or
other services that may affect function or reliability, or when it deviates with a standard. The seller shall notify Electrolurgy, Inc. of changes to
facility location, ownership, quality management or downgrade of quality system following a quality survey by Electrolurgy, Inc - SHELF-LIFE CONTROL: For age sensitive commodities, the materials provided shall have as a minimum 75 percent of unused shelf-life
from the date of shipment, unless approved otherwise Electrolurgy, Inc. The product name, batch number, manufacturing date, expiration
statement/date, storage requirement, shall be noted on the container and the certificate of compliance. - GOVERNMENT RATED ORDER
- ISO 9001 COMPLIANCE: This contract requires compliance to ISO 9001 Quality Management System, or other industry recognizable
system, applicable to the supplier. Compliance to the Electrolurgy, Inc. survey may be used in lieu of the above requirements - MSDS: Supplier agrees to furnish the applicable Material Safety Data Sheet(s) (MSDS) with each shipment for product designated by
industry, state or federal agencies as hazardous material. - SHIPPING AND PACKAGING: Packaging, labeling and protection of the product shipped shall be in accordance with Electrolurgy, Inc.
Purchase Order or best commercial practice to protect the goods and finish applied from damage and deterioration. - 100% INSPECTION: Perform 100% inspection on characteristics specified by Electrolurgy, Inc. Sampling inspection is not allowed. A copy of
this inspection is required with shipment of product. - SUBTIER FLOW-DOWN: The suppliers shall flow down the applicable Electrolurgy, Inc. quality clauses to sub-tier suppliers
- FIRST-ARTICLE INSPECTION: Verify all characteristics specified on First Piece and all other parts must be held until the first piece is
inspected and approved during inspection. If the first piece is acceptable, it shall be identified as :FIRST PIECE and the inspection records
shall be delivered to Electrolurgy, Inc. with shipment - SAMPLING PLAN: Sampling inspection may be used and must conform to ANSI/Z1.4 (Obsolete spec MIL-STD-105) or as otherwise
specified by the applicable specification. - SOURCE INSPECTION: When this quality clause is invoked on the P.O., an Electrolurgy, Inc., Government or customer source inspection is
required prior to shipment of acquired article(s). The supplier is responsible for providing the inspectors with verifiable evidence of quality,
reasonable facilities and inspection tools, copies of drawings, specifications as well as the necessary assistance needed to perform the task.
The supplier shall promptly notify Electrolurgy, Inc. or its representatives when the article is ready, so that the appropriated planning for
inspection can be accomplished. Such inspection shall be deemed as preliminary and all material ordered shall be subject to final inspection
and acceptance at Electrolurgy, Inc. - SERIALIZATION: Serialization requirements are imposed. All parts have unique serial numbers assigned. Maintain S/N traceability at all
stages of manufacturing processing. Indicate S/N on all certifications, shipping reports and/or documents - FINAL ACCEPTANCE OF HARDWARE: Final Source inspection is required by Electrolurgy, Inc. Notify Electrolurgy Quality Assurance
department within 48 hours prior to product being ready for inspection. Electrolurgy will coordinate inspection with Electrolurgy customer
and/or regulatory agency. - NON-CONFORMING MATERIAL: If a non-conformance is detected during the inspection, manufacture, or test, the supplier shall control the
nonconforming item to assure that it is identified, isolated and quarantined. The supplier shall notify Electrolurgy, Inc. of the non-conformance
within a reasonable time, preferably within 48 hours of discovery, or arrange for Electrolurgy disposition of non-conforming material. - QUALIFIED PERSONNEL: Certain operations are required to be performed by trained or qualified personnel
- NDT SUPPLIERS: Qualified and Approved by Prime Contractor Supplier, Qualified Personnel, Qualified Products, Process Control, control of
Quality Records, Indication of Inspection Status and adequate work instructions are required. NDT stamp shall appear on parts or a tag
attached to the part. Certification of Conformance must include name and certification level of NDT personnel that performed the inspection or
is responsible for NDT inspection at Supplier.
